Report Your Child's Immunization Record to Ottawa Public Health

Ottawa Public Health (OPH) and CANImmunize are pleased to announce an app for residents of Ottawa to securely report their children's immunization records to Ottawa Public Health using the CANImmunize app on iOS and Android devices.

Eligibility Criteria

  • You must be using the CANImmunize app on an iPhone or iPad running iOS or a smartphone or tablet running Android.
  • Your child must be less than 16 years of age.
  • Your device must be connected to the internet.
  • Your child must live in the City of Ottawa.

Residents within the following City of Ottawa regions are eligible to partake in the Ottawa Public Health initiative:

Arnprior, Ashton, Carlsbad Springs, Carp, Cumberland, Dunrobin, Edwards, Fitzroy Harbour, Gloucester, Greely, Kanata, Kars, Kenmore, Kinburn, Manotick, Metcalfe, Munster, Navan, Nepean, North Gower, Orleans, Osgoode, Ottawa, Pakenham, Ramsayville, Richmond, Rockcliffe, Sarsfield, Stittsville, Vanier, Vars, Vernon, Woodlawn

Instructions

Step 1: Get the App

Download on your iOS device or Android device.

Step 2: Navigate to your child's profile in CANImmunize

To begin, tap on the 'My Records' button on the home screen of the app. If you have already created a record for your child, select it. If not, tap on 'Create New Record' and follow the instructions. When you get to the City / Town field, proceed to Step 2.

If you have an existing record you will see a list of your child’s visits (Two Month Visit, Four Month Visit, etc...). Tap on the ‘Profile’ button in the bottom right hand corner of the screen to return to your child’s profile page.

Step 3: Set the City / Town field to Ottawa

Once on your child’s profile page, scroll down to the 'City / Town' field. Ensure that your child's 'City / Town' field is set to 'Ottawa' or one of the regions from this list. Once you have selected the correct city, tap the 'Save' button in the top right hand corner of the screen.

Step 4: Register for an account

Once you have properly set the city field, you will see the option to 'Send to Ottawa Public Health'. Tap the switch next to this text to ‘on’.

You will now be on a page that has two buttons: 'Register' and 'Login'. Tap 'Register', enter your email address and create a password. 

You will need to enter your password again to confirm that you have entered it properly. Press 'Next'.

On the next page, enter the consenter (parent / guardian) first and last name and then press 'Register'.

If you forget your password at any point, you will be able to reset it by entering the email and the exact consenter last name used to register the account.

Step 5: Enable Sending to OPH feature for your child

You will now be on a page with a list of all of your immunizations recorded on CANImmunize. To select a record, tap the switch next to the name of the child whose immunization records you would like to send to Ottawa Public Health.

Enter your child's OHIP number (without the version code). Verify that the child's 'City / Town' selection is correct, and then press 'Next'.

Enter your contact information and then press 'Next'.

Please read the consent message, and then press 'I Consent'. Your child's record will now be transmitted to Ottawa Public Health. This may take a few seconds but you can verify that your child's record was received by ensuring that there is a green checkmark (insert green checkmark) under their name in the list of your records.

Changing Ottawa Public Health Settings & Logging Out

You may access the Ottawa Public Health settings from the CANImmunize main menu by tapping three buttons in the top left hand corner of the home screen. The Ottawa Public Health menu button will be visible as long as at least one of your records' City / Town fields is set to Ottawa or a region within Ottawa Public Health's catchment.

FAQs

What is the Send Updates Automatically setting for?

If the Send Updates Automatically setting is on, any record that is transferrable to OPH will be updated and sent automatically. A message will appear on the screen each time the record is transmitted and you will be able to verify that the record was sent by checking the 'Last Updated with OPH' date on the user's profile or in the OPH Settings.

If you choose to disable automatic transmission, you will be required to tap the 'Send to Ottawa Public Health' button in the record's profile or in the OPH Settings when you would like to communicate any updates to Ottawa Public Health.

How do I stop sending my records to OPH?

You can stop sending your child's immunization record to OPH by turning off the 'Send to Ottawa Public Health' switch in their profile. Alternatively, you may stop sending all of your records at once by going to the Ottawa Public Health Settings page from the app's main menu and logging out.

I am having trouble reporting my child's immunization record. Who can I contact?

You can contact CANImmunize support by submitting feedback or reporting a bug in the app.

Why do I need to report my child's immunization information?

As a resident of Ottawa, it is required by law under the Immunization of School Pupils Act (ISPA) that you report your child's immunization information to Ottawa Public Health.

How do I report my child's immunization information without using CANImmunize?

Ottawa Public Health requires that you complete their Student Immunization Information Form found on their website and submit it by e-mail or fax, or report by calling OPH with the information.
